We hate forms as much as you do and we do our best to keep them to a minimum as much as possible. So let's be right up front, we have forms; we are sorry about that. However, these forms are required by law, but more importantly, it's good practice for us to provide you with this information and answer all your questions as we begin to build a relationship with you. You are entrusting us with some tough stuff, and we want to make sure you know how and what we do with that information. What you tell us is confidential with a few exceptions that we spell out in our forms. We also want you to know what you can expect from us, what our fees are for the numerous services we offer and our backgrounds; again we want to be right up front with you.
As of 2016, this office began using an electronic medical record system. The good news for you is your medical records are more secure than ever. Your records are stored in an HIPPA manner in an off-site storage facility. No personal data is stored on our computer terminals.
Additionally, the intake form is all online. Please provide us with your email address and we will send you the link before your scheduled appointment. If you do not have an email, you can fill out the paperwork online in our office. Another bonus of electronic records is the forms are slightly shorter, yea!
